Assessment is functional — strength, gait speed, balance, and performance — rather than based on mass alone, because muscle mass and muscle function separate with age and separate again under treatment. Several pharmacological programmes in this area have produced increases in lean mass with no improvement in what patients could do, which is a negative result reported in flattering units. The investigational work targets the neuromuscular junction and the regulatory brakes on muscle maintenance rather than bulk as an end in itself.
