Assessment extends past fasting glucose and HbA1c to dynamic measures — insulin sensitivity, substrate utilisation, metabolic flexibility, and quantified hepatic fat — because interventions here are selected against the specific defect rather than against a diagnostic label. Two patients carrying the same diagnosis frequently have different dominant abnormalities, and treating them identically is what produces the inconsistent results this area is known for.
