Alcohol, opioid, and nicotine use disorder, with an emphasis on patients for whom approved pharmacotherapy and structured behavioural treatment have not produced durable remission.
Relapse is driven by circuit-level change that outlasts detoxification by years, which is why agents effective against acute withdrawal do not prevent return to use. Approved treatment is offered and optimised first; investigational approaches are restricted to those who have exhausted it. That sequence is not negotiable, and it is the condition under which this work is defensible at all.
